Bilingual RN Care Team

Job Summary

Bilingual RN Care Team members will provide exceptional care and health education to patients remotely, supporting chronic condition management and creating personalized action plans while coordinating with clinical team members.

Key responsibilities

• Provide disease management and health education to patients
• Create personalized action plans and support goal setting for individual patients
• Conduct regular outreach to patients to enhance engagement and elicit behavior change

Required qualifications

• Compact state license required; additional licensing may be needed
• Bilingual (Spanish speaking, reading, and writing) required
• 2+ years of experience as a Licensed Practical Nurse or any Nursing license
• 2+ years of experience in patient-facing or customer-facing roles
• Experience working in telehealth or healthcare startup environments preferred
