Bilingual RN Case Manager

Job Summary

A company is looking for a Bilingual Clinical Guide: Case Manager (Gold Care).

Key Responsibilities

• Develop long-term patient relationships and conduct holistic assessments to identify patient needs
• Create and evaluate care plans in collaboration with interdisciplinary teams and caregivers
• Support chronic disease management and ensure smooth transitions of care for patients

Required Qualifications

• An unrestricted compact RN license
• RN Complex Case Management experience
• A minimum of 5 years of direct clinical experience in acute care
• Strong knowledge of interdisciplinary teamwork
• Comfort with participating in interdisciplinary discussions
