Bilingual RN Case Manager

Job Summary

A company is looking for a Case Manager RN - Level I - Bilingual.

Key Responsibilities

• Manage an individual caseload using the Case Management process
• Coordinate care transitions and collaborate with healthcare providers and teams
• Participate in quality improvement initiatives and maintain documentation compliance

Required Qualifications

• Current active and unrestricted Registered Nurse licensure in the United States
• Bachelor's degree in a health-related field preferred
• 7 years of varied clinical experience preferred
• Must be fluent in English and Spanish
• URAC-recognized certification in Case Management within 3 years of hire
