Job Summary

Key Responsibilities

Engage with newly discharged patients to assess their needs and develop care plans
Coordinate care with healthcare providers and community resources to ensure comprehensive support
Educate patients on managing chronic conditions and navigating the healthcare system

Required Qualifications

Unrestricted Compact RN license obtained in the United States
3-4 years of RN Care Management or telephonic case management experience
Fully bilingual in English and Spanish
Availability to work Monday-Friday during specified hours
