Bilingual RN Case Manager

Job Summary

Managing a full caseload remotely, the full-time Bilingual RN Case Manager will coordinate care transitions, review treatment plans, and communicate with healthcare teams while ensuring quality patient care.

Key responsibilities

• Independently manage a full caseload and coordinate care transitions and discharge planning
• Collaborate with providers, social workers, and insurance teams to ensure comprehensive patient care
• Participate in quality improvement initiatives and maintain accurate documentation of case actions

Required qualifications

• Current active and unrestricted Registered Nurse licensure in the United States
• Bachelor's degree in a health-related field preferred
• Seven years of varied clinical experience preferred
• Fluency in English and Spanish with strong reading and writing abilities in both languages
• URAC-recognized certification in Case Management within three years of hire
