Bilingual Russian or Chinese Care Coordinator

Job Summary

A company is looking for a Care Coordinator who is fluent in Russian or Chinese.

Key Responsibilities:

• Manage a monthly allocation of patients and provide patient-centered care remotely
• Optimize the company's Chronic Care Management and Remote Patient Monitoring platforms
• Create and update individual Care Plans for patients and communicate with their doctors

Required Qualifications:

• Certified/Registered Medical Assistant, LPN/LVN, RN/BSN credentials
• Familiarity with Chronic Care Management and/or Remote Patient Monitoring is a plus
• Efficient in working independently from a HIPAA compliant space at home
• Fluent in Russian or Chinese language
• Excellent time management skills and commitment to patient care
