Job Summary

Overseeing a team of Care Coordinators, the full-time remote Bilingual Spanish Care Coordination Supervisor will manage care coordination programs, ensure compliance with health care management requirements, and support the development of individualized care plans for members with complex needs.

Key responsibilities

• Manage and mentor the Care Coordinator team to ensure effective delivery of care and compliance with program requirements
• Develop and oversee care coordination programs, workflows, and standard operating procedures
• Collaborate with management to improve processes and ensure timely completion of health assessments and care plans

Required qualifications

• 3+ years of experience in Care Coordination or a related field
• 1+ years of supervisory experience in a health care setting
• High school diploma or GED; equivalent education and experience may be considered
• Knowledge of Managed Care Plans and medical terminology
• Ability to analyze data and identify trends
