Bilingual Spanish Care Coordinator

Job Summary

A company is looking for a Care Coordinator, SNP (Bilingual Spanish).

Key Responsibilities

• Assist members with referrals, authorizations, and care coordination needs
• Create cases and complete assessments for hospital and SNF discharges
• Collaborate with the RN Case Manager to manage a panel of SNP members

Required Qualifications, Training, and Education

• Minimum 1 year experience in Health Care IPAs and entering referrals/authorizations
• High School Diploma or GED; Bachelor's degree or four years additional experience in lieu of education
• Preferred: Medical Assistant training and Medical Terminology training
• Bilingual: Spanish or Vietnamese is preferred
• No specific licensure required
