Bilingual Spanish Care Coordinator

Job Summary

A company is looking for a Bilingual Spanish Care Coordinator for Outpatient Case Management.

Key Responsibilities

• Assist members with referrals, authorizations, and scheduling appointments
• Create and document cases in the case management module for hospital and skilled nursing facility discharges
• Work collaboratively with the case manager to manage member care and maintain updated records

Required Qualifications, Training, and Education

• Minimum of 1 year experience in health care, such as a health plan or medical office
• Minimum of 1 year experience assisting members with authorizations and scheduling
• High School Diploma or GED required; Bachelor's degree preferred or equivalent experience
• Medical assistant training and medical terminology training preferred
• Bilingual in English and Spanish preferred
