Bilingual Spanish Care Navigator

Job Summary

Seeking a compassionate Bilingual Spanish Care Navigator, the full-time remote position will manage patient care coordination, handle inbound communications, and provide support to patients and caregivers while working within a dedicated clinical team.

Key responsibilities

• Answer inbound calls, emails, and text messages from patients, caregivers, and external providers
• Schedule appointments and maintain ongoing contact with patients to coordinate care
• Document patient and caregiver needs while providing emotional support and educational resources

Required qualifications

• Experience in care coordination or navigation, preferably with cognitive challenge populations
• Ability to provide a personal computer/laptop that meets security and telehealth standards
• Strong time management and communication skills
• Empathy and patience in dealing with patients and caregivers
• Understanding of health insurance benefits and eligibility processes
