Bilingual Spanish LPN

Job Summary

A company is looking for a Bilingual/Spanish Speaking Licensed Practical Nurse.

Key Responsibilities

• Conduct outbound telephonic patient outreach to support patients enrolled in care programs
• Provide one-on-one coaching and support to patients managing chronic conditions
• Document patient interactions and collaborate with the clinical team to ensure comprehensive care

Required Qualifications

• An active compact multi-state LPN/LVN license in the state of residence is required
• At least 5 years of clinical experience as a Licensed Practical Nurse is required
• Fluency in Spanish is required
• Remote work readiness, including stable high-speed WiFi, is required
• Experience providing patient education and support for chronic conditions is preferred
