Bilingual Spanish or Creole Care Transition Case Manager

Job Summary

A company is looking for a Care Transition Case Manager.

Key Responsibilities:

• Engaging with newly discharged members telephonically to understand their needs and conducting holistic assessments
• Developing care plans in partnership with members and their caregivers, educating members on chronic conditions, and providing active care management
• Working with other providers and resources, referring members to Devoted Medical Group, coordinating post-treatment care, and collaborating with PCP partners

Required Qualifications:

• Unrestricted Compact RN license obtained in the United States with the ability to secure other licensure states
• Minimum of 4 years of RN experience in an acute setting
• Telephonic case management experience
• Bilingual in Spanish or Creole
• Availability to work 8:00am - 5:30pm MST or CST
