Bilingual Spanish Patient Navigator

Job Summary

A company is looking for a Bilingual Spanish Patient Navigator.

Key Responsibilities:

• Serve as the patient's liaison throughout the program, addressing quality measures and adhering to company guidelines
• Outreach to patients' providers, input appointments, and provide regular outreaches to the patient
• Identify and connect patients to resources for addressing Social Determinants of Health (SDOH)

Required Qualifications:

• High School Diploma / GED (or higher)
• 2+ years of customer service experience
• Bilingual fluency in English and Spanish
• Experience with Microsoft Word, Excel, and Outlook
• Experience working within the health care industry and/or with health care insurance
