Bilingual Spanish RN Care Manager

Job Summary

To enhance member health outcomes, the full-time Bilingual Spanish RN Care Manager will provide telephonic care coordination, collaborate with clinical teams, and develop personalized care plans while working remotely in the USA.

Key responsibilities

• Engage in telephonic care coordination and management for identified patient populations
• Collaborate with Clover clinical teams to facilitate member assessments and care planning
• Document member care activities and advocate for members' needs and choices

Required qualifications

• At least 3 years of post-licensure care management experience
• Bachelor's degree (BSN) or higher from an accredited nursing school
• Current valid RN multi-state license
• Fluency in English and Spanish, both orally and in writing
• Experience with assessment, care plans, and care coordination
