Bilingual Spanish Telephonic Care Coordinator

Job Summary

A company is looking for a Bilingual Spanish Telephonic Care Coordinator.

Responsibilities:

• Conduct monthly phone calls with patients, practices, pharmacies, etc. to ensure patients' needs are met
• Create and revise personalized care plans for each patient in coordination with practices and providers
• Identify social determinants of health, gaps in care, and eligibility for assistance or referral services

Qualifications:

• 1-2 years healthcare experience as a Certified/Registered Medical Assistant with active accreditation
• Technology experience, including familiarity with multiple EMR/EHR systems and Microsoft Office
• Highly professional and compassionate phone demeanor with strong critical thinking skills
• Highly self-motivated, organized, and able to work independently
• Must have a home office setup with a second monitor and fast, reliable internet (50mbps or greater)
