Bilingual Transition of Care Nurse

Job Summary

A company is looking for a Bilingual Transition of Care Nurse.

Key Responsibilities

• Outreach to caregivers and patients after hospital discharge for Transition of Care visits
• Coordinate care between patients and their PCP/specialists post-discharge to ensure continuity of care
• Provide anticipatory guidance and education to families while assessing barriers to care

Required Qualifications

• Bachelor's in nursing from an accredited university required
• Compact Licensed RN, or eligibility to register for other state licenses
• Minimum of 2 years direct clinical experience in Emergency Department/Triage, Complex Care, or PICU preferred
• Experience with pediatric population required
• Bilingual in English and Spanish preferred
