Bilingual Transitions of Care APP

Job Summary

To support a growing Transitions of Care program, the full-time Bilingual Transitions of Care APP will provide virtual clinical care for patients post-discharge, conducting assessments, developing care plans, and collaborating with an interdisciplinary team while working remotely within MST/PST hours.

Key responsibilities

• Perform initial and acute symptom TOC visits, including comprehensive assessments and management of patient care
• Participate in interdisciplinary TOC rounds to discuss high-risk patients and enhance team communication
• Utilize electronic health information systems and contribute to quality improvement projects

Required qualifications

• At least 3 years of experience in hospital, post-hospital, or primary care settings
• Bilingual in Spanish and English
• Active RN and APRN licenses or Physician Assistant license in good standing
• Ability to work full-time, five 8-hour days per week (Monday-Friday, 8:30 AM - 5:00 PM MST/PST)
• Strong team player with a focus on interdisciplinary collaboration
