Job Summary

A company is looking for a Bilingual Transitions of Care Guide.

Key Responsibilities:

Calling members after discharge to check in and provide necessary assistance
Coordinating services and care for members, including transportation and home health care
Assisting with scheduling post discharge PCP and Specialist visits

Required Qualifications:

High school diploma or GED equivalent
Fully bilingual in English and Spanish
2+ years in healthcare setting, managed care, home health, or community health highly desired
Associates degree in healthcare related field preferred
