Job Summary

A company is looking for a Bilingual (Spanish) Transitions of Care Nurse Practitioner.

Key Responsibilities

• Perform initial transitions of care visits, including comprehensive assessments and medication reconciliation
• Conduct virtual urgent care visits to diagnose and manage acute symptoms
• Participate in interdisciplinary rounds to discuss high-risk patients and enhance team culture

Required Qualifications

• At least 3 years of experience in a hospital, post-hospital, or primary care setting
• Bilingual in English and Spanish
• Must be willing to work five 8-hour shifts (Monday-Friday)
• Active RN and APRN licenses in good standing
• Active BLS certification at the time of hire
