Bilingual Vietnamese Speaking Care Coordinator

Job Summary

A company is looking for a Bilingual Vietnamese Speaking Care Coordinator.

Key Responsibilities:

• Manage a monthly allocation of patients and provide patient-centered care through telephonic interactions
• Create and update individual Care Plans for patients, ensuring their doctors have up-to-date information
• Coordinate with specialists, labs, and community services, and encourage preventative health services

Required Qualifications:

• Fluent in Vietnamese and English
• Clinical credentials such as MA, LPN, RN, BSN, or similar
• Minimum 1-year experience in a clinical setting or patient environment
• Ability to navigate multiple computer systems and handle PHI securely
• HIPAA compliant area in home to conduct Virtual Care Services and high-speed internet
