Job Summary
To support high-risk patient care initiatives, the full-time salaried California and Washington Licensed Director of Population Outcomes will develop and manage strategies to address utilization and high-cost medical expenses while collaborating with interdisciplinary teams in a fully remote environment.
Key responsibilities
- Identify and analyze utilization trends, partnering with interdisciplinary teams to plan interventions
- Perform population surveillance to early identify rising risk patients and implement care plans to prevent future utilization
- Design and manage projects with key leaders to ensure effective implementation and tracking of outcomes
Required qualifications
- Graduation from an accredited nurse practitioner school
- Master's Degree required
- Board Certified Nurse Practitioner with licenses in CA and/or WA or the ability to obtain immediate licensing
- Five or more years of clinical experience, including at least three years in a value-based care environment
- Demonstrated expertise in reporting, analyzing, and designing initiatives based on data analysis
