Key Responsibilities

• Oversee monitoring of patient vitals, symptoms, and labs in collaboration with the healthcare team
• Manage treatment plans and medications, focusing on guideline-directed medical therapy for chronic conditions
• Conduct regular virtual check-ins with patients to review labs and optimize treatment adherence

Required Qualifications

• 5+ years of experience treating patients with chronic diseases (T2D, Hypertension, CHF)
• Active multi-state compact RN license and NP licenses in CA and WA
• ANCC or AANP certification required
• Master's Degree as a Nurse Practitioner
• Experience managing CHF with guideline-directed medical therapy
