Job Summary
Seeking a highly motivated full-time California Bilingual Case Manager, who will engage with individuals facing Serious Mental Illness/Substance Use Disorder and high medical needs, primarily through remote interactions while working Pacific Time hours.
Key responsibilities
- Maintain an ongoing caseload using evidence-based approaches to promote health goal achievement
- Utilize relationship-based strategies for social support navigation and to build trust with clients
- Conduct telephonic and SMS outreach for timely follow-ups and assist in coordinating care management appointments
Required qualifications
- 1+ years of professional experience in case management, care coordination, or healthcare call center
- Bilingual in English and Spanish
- Strong understanding of cultural fluency and ability to build trust with diverse populations
- Demonstrated ability to work collaboratively in a multidisciplinary care team
- Reliable workspace with a stable internet connection for documentation and communication
