Job Summary

Seeking a full-time California Bilingual Healthcare Representative, the role focuses on engaging individuals with serious mental illness or high medical needs through telephonic outreach and support, while working primarily remote but within Pacific Time hours.

Key responsibilities

• Maintain an ongoing caseload using evidence-based approaches to promote member engagement and health goal achievement
• Conduct telephonic and SMS outreach to ensure timely follow-up and support for members navigating social services
• Collaborate with multidisciplinary care teams to address care issues and meet monthly encounter metrics for compliance

Required qualifications

• 1+ years of professional experience in case management, care coordination, or a healthcare call center setting
• Bilingual in English and Spanish
• Strong understanding of cultural fluency and ability to build trust with diverse populations
• Excellent organizational skills and proficiency with tools such as Google Suite and CRM databases
• Reliable, HIPAA-compliant workspace with a stable internet connection
