Job Summary
To support a growing direct service practice, the full-time salaried California Care Navigator will build and maintain relationships with transition-age youth (TAY) and kinship families in California, providing ongoing support and navigation through complex social service systems while working remotely.
Key Responsibilities
- Serve as a consistent point of contact for TAY and kinship families, building trust and facilitating engagement through various communication methods
- Conduct comprehensive needs assessments and develop personalized action plans while monitoring progress and adapting strategies as needed
- Document insights from direct interactions to inform service improvements and contribute to the continuous development of the digital navigation practice
Required Qualifications
- 3+ years of experience navigating child welfare or social service systems, with at least 2 years in a direct service or navigation role
- Trained in Motivational Interviewing, with a focus on trauma-informed and culturally responsive practices
- Familiarity with California's child welfare system and relevant state programs
- Experience with data entry and documentation in case management or tracking systems
- Bachelor's Degree in social work, counseling, human services, or a related field, or equivalent lived experience
