California Community Health Worker Manager

Job Summary

Leading a team of CalAIM Community Health Workers, the full-time salaried California Community Health Worker Manager will oversee service delivery, ensure program compliance, and facilitate training while working remotely from California.

Key Responsibilities

• Oversee, mentor, and evaluate a team of CalAIM CHWs conducting outreach and advocacy for high-risk populations
• Maintain compliance with CalAIM guidelines, auditing CPT coding and enforcing service unit limits
• Coordinate onboarding and continuing education for CHWs, including training in Motivational Interviewing and Mandated Reporting

Required Qualifications

• Bachelor's degree in Public Health, Social Work, Healthcare Administration, or a related field preferred
• 3-5 years of experience in community health, Medi-Cal services, or social work, with at least 2 years in a supervisory role
• Strong expertise in CalAIM policies and social determinants of health
• Valid California Driver's License (Class C) with an operable vehicle and clean MVR
• CHW certification preferred
