Job Summary

A company is looking for a Remote Admissions Coordinator Long-Term Care & Rehabilitation (RN or LVN/LPN).

Key Responsibilities
  • Coordinate and oversee admissions and transitions of care for members entering long-term care or rehabilitation settings
  • Review clinical documentation to support appropriate placement based on member needs and Medi-Cal criteria
  • Serve as the primary liaison between health plans, SNF admission teams, and hospital discharge planners to ensure timely placements
Qualifications
  • Active, unrestricted California RN or LVN/LPN license (required)
  • Minimum of 2-3 years of experience in home health, long-term care, or intake/placement coordination
  • Working knowledge of California Medi-Cal and long-term care guidelines
  • Proficiency with EMRs and care coordination platforms
  • Ability to work independently in a fully remote environment
