Job Summary

A company is looking for a Nurse Practitioner OR Physician Assistant - Advanced Care Partner.

Key Responsibilities

• Maintain task category SLAs and engage in clinical dialogue with patients
• Manage medication changes and provide follow-up care through video visits and phone consultations
• Collaborate with PCPs and participate actively in the care team for complex patient management

Required Qualifications

• 5+ years of experience as a Nurse Practitioner or Physician Assistant
• Proficiency in remote/virtual medical care
• Experience in chronic disease care management and patient education
• Current CA license is mandatory
• Expertise in managing high-risk patient caseloads
