Job Summary

A company is looking for a Nurse Practitioner OR Physician Assistant - Advanced Care Partner.

Key Responsibilities

• Collaborate with PCPs to manage complex patient panels and maintain task category SLAs
• Engage in clinical dialogue with patients, provide medication management, and conduct follow-up care
• Participate actively in the care team, including real-time responses and case presentations

Required Qualifications

• 5+ years of experience as a Nurse Practitioner or Physician Assistant
• Proficient in remote/virtual medical care and chronic disease management
• Expertise in managing high-risk patient caseloads and metabolic diseases
• Current CA license is mandatory
• Experience with patient education and data analysis (CGM and EKG)
