Key Responsibilities

• Maintain task category SLAs, including answering clinical questions and engaging in clinical dialogue
• Manage medication changes and provide follow-up care through video visits and phone consultations
• Participate actively in the care team, including real-time communication and case presentations

Required Qualifications

• 5+ years of experience as a Nurse Practitioner or Physician Assistant
• Proficiency in remote/virtual medical care
• Experience in chronic disease management and patient education
• Current licensure in California
• Expertise in managing high-risk patient caseloads
