Job Summary
To support the Transitions of Care Program, the full-time California Licensed Advanced Practice Provider will provide virtual clinical care post-discharge, conducting assessments, developing care plans, and collaborating with an interdisciplinary team to prevent patient readmissions.
Key responsibilities
- Perform initial post-discharge visits, including comprehensive assessments and medication reconciliation
- Conduct virtual urgent care visits to address acute symptoms and manage patient care
- Participate in interdisciplinary rounds to discuss high-risk patients and enhance team communication
Required qualifications
- At least 3 years of experience in a hospital, post-hospital, or primary care setting
- Active RN and APRN or Physician Assistant licenses in good standing
- Ability to work full-time, five days a week, during specified hours
- Strong team player with a focus on interdisciplinary collaboration
- Active BLS certification at the time of hire
