Job Summary

A company is looking for an Ambulatory Care Specialist.

Key Responsibilities
  • Provide medication management, including reconciliation and adherence counseling
  • Collaborate with care teams to develop individualized care plans based on evidence-based guidelines
  • Monitor medication regimens to ensure therapeutic goals and clinical criteria are met
Required Qualifications, Training, and Education
  • Doctor of Pharmacy (PharmD) and licensed pharmacist in California
  • Advanced practice pharmacist certification or completion of a postgraduate residency
  • 1,500 hours of clinical experience under a collaborative practice agreement within the last 10 years is preferred
  • Completion of PGY1/PGY2 residency is preferred but not required
  • Strong knowledge of pharmacology and FDA health regulations
