California Licensed Ambulatory Care Pharmacist

Job Summary

A company is looking for an Ambulatory Care Pharmacist to provide integrated support to patients and physicians, improving medication adherence and promoting evidence-based care.

Key Responsibilities

• Provide medication management, including reconciliation, dose optimization, and adherence counseling
• Collaborate with the interdisciplinary care team to develop individualized care plans based on evidence-based guidelines
• Monitor and evaluate medication regimens to ensure clinical criteria and therapeutic goals are met

Required Qualifications and Education

• Doctor of Pharmacy (PharmD) and a licensed pharmacist in California
• Must meet two of the three criteria for advanced practice pharmacist in California
• Completion of PGY1/PGY2 residency is preferred but not required
• 1,500 hours of clinical experience under a collaborative practice agreement within the last 10 years is required
• Strong clinical knowledge and expertise in disease state management and treatment guidelines
