Job Summary

A company is looking for an Ambulatory Care Pharmacist to provide integrated support to patients and physicians for improved medication management and patient care.

Key Responsibilities
  • Provide medication management, including reconciliation, dose optimization, and adherence counseling
  • Collaborate with the interdisciplinary care team to develop individualized care plans based on evidence-based guidelines
  • Monitor medication regimens to ensure clinical criteria are met and identify opportunities for drug therapy optimization
Required Qualifications and Education
  • Doctor of Pharmacy (PharmD) and a licensed pharmacist in California
  • Must be an advanced practice pharmacist in California, meeting two of three specified criteria
  • Completion of a postgraduate residency is preferred but not required
  • 1,500 hours of clinical experience under a collaborative practice agreement within the last 10 years is required
  • Strong clinical knowledge and expertise in disease state management and treatment guidelines
