California Licensed Ambulatory Clinical Pharmacist

Job Summary

A company is looking for an Ambulatory Clinical Pharmacist (CA License Required).

Key Responsibilities

• Provide medication management, including reconciliation, dose optimization, and adherence counseling
• Collaborate with the interdisciplinary care team to develop individualized care plans based on evidence-based guidelines
• Monitor and evaluate medication regimens to ensure compliance with clinical criteria and therapeutic goals

Required Qualifications, Training, and Education

• Doctor of Pharmacy (PharmD) and a licensed pharmacist in CA
• Must meet two of the three criteria for advanced practice pharmacist status in CA
• Completion of PGY1/PGY2 residency is preferred but not required
• 1,500 hours of clinical experience under a collaborative practice agreement within the last 10 years is required
• Strong clinical knowledge and expertise in disease state management and treatment guidelines
