Job Summary
To enhance patient care quality, the part-time California Licensed Ambulatory Clinical Pharmacist will provide medication management, collaborate with interdisciplinary teams, and conduct clinical assessments remotely while focusing on medication adherence and therapy management.
Key Responsibilities
- Provide medication management including reconciliation, dose optimization, and adherence counseling
- Collaborate with healthcare providers to develop individualized care plans and ensure seamless medication management
- Monitor and evaluate medication regimens to meet clinical criteria and therapeutic goals
Required Qualifications
- Doctor of Pharmacy (PharmD) and a licensed pharmacist in California
- Advanced practice pharmacist certification or completion of a postgraduate residency
- 1,500 hours of clinical experience under a collaborative practice agreement within the last 10 years
- Proficiency in Microsoft Office tools and web-based platforms
- Strong clinical knowledge of pharmacotherapy and disease state management
