Job Summary

To enhance patient care quality, the part-time California Licensed Ambulatory Clinical Pharmacist will provide medication management, collaborate with interdisciplinary teams, and conduct clinical assessments remotely while focusing on medication adherence and therapy management.

Key Responsibilities
  • Provide medication management including reconciliation, dose optimization, and adherence counseling
  • Collaborate with healthcare providers to develop individualized care plans and ensure seamless medication management
  • Monitor and evaluate medication regimens to meet clinical criteria and therapeutic goals
Required Qualifications
  • Doctor of Pharmacy (PharmD) and a licensed pharmacist in California
  • Advanced practice pharmacist certification or completion of a postgraduate residency
  • 1,500 hours of clinical experience under a collaborative practice agreement within the last 10 years
  • Proficiency in Microsoft Office tools and web-based platforms
  • Strong clinical knowledge of pharmacotherapy and disease state management
