Job Summary

A company is looking for an Ambulatory Clinical Pharmacist with a CA License.

Key Responsibilities
  • Provide medication management, including reconciliation, dose optimization, and adherence counseling
  • Collaborate with the interdisciplinary care team to develop individualized care plans based on evidence-based guidelines
  • Monitor and evaluate medication regimens to ensure clinical criteria are met and participate in quality improvement initiatives
Required Qualifications, Training, and Education
  • Doctor of Pharmacy (PharmD) and a licensed pharmacist in California
  • Must meet two of the following: certification in relevant practice area, completion of a postgraduate residency, or 1,500 hours of clinical experience under a collaborative practice agreement
  • Completion of PGY1/PGY2 residency is preferred but not required
  • Strong clinical knowledge and validated expertise in disease state management and treatment guidelines
  • Ability to maintain confidentiality and manage sensitive data effectively
