California Licensed Ambulatory Pharmacist
Job Summary
A company is looking for an Ambulatory Care Pharmacist who will enhance patient care through comprehensive medication management.
Key Responsibilities
- Provide comprehensive medication management including reconciliation, dose optimization, and adherence support
- Collaborate with healthcare providers to develop individualized care plans and ensure medication coordination
- Monitor therapy effectiveness, identify optimization opportunities, and support quality improvement initiatives
Required Qualifications
- Doctor of Pharmacy (PharmD) degree
- Licensed pharmacist in California
- Certification in a relevant field of practice or completion of a postgraduate residency
- At least one year of clinical service experience under a collaborative practice agreement
- 3+ years of experience as a clinical pharmacist or completion of PGY2
