Job Summary

A company is looking for an Ambulatory Care Pharmacist who will enhance patient care through comprehensive medication management.

Key Responsibilities
  • Provide comprehensive medication management including reconciliation, dose optimization, and adherence support
  • Collaborate with healthcare providers to develop individualized care plans and ensure medication coordination
  • Monitor therapy effectiveness, identify optimization opportunities, and support quality improvement initiatives
Required Qualifications
  • Doctor of Pharmacy (PharmD) degree
  • Licensed pharmacist in California
  • Certification in a relevant field of practice or completion of a postgraduate residency
  • At least one year of clinical service experience under a collaborative practice agreement
  • 3+ years of experience as a clinical pharmacist or completion of PGY2
