Job Summary

Key Responsibilities
  • Provide comprehensive medication management, including reconciliation and dose optimization

  • Monitor therapy effectiveness, identify optimization opportunities, and support quality improvement initiatives
Required Qualifications, Training, and Education
  • Doctor of Pharmacy (PharmD) degree
  • Licensed pharmacist in California
  • Advanced practice pharmacist certification in California, meeting at least two specified criteria
  • 3+ years of experience as a clinical pharmacist or completion of PGY2 residency
  • Experience in clinical settings involving drug therapy management
