Job Summary
A company is looking for an Ambulatory Care Pharmacist who will enhance patient care through specialized medication management.
Key Responsibilities
- Provide comprehensive medication management, including reconciliation and dose optimization
- Collaborate with healthcare teams to create individualized care plans and ensure medication coordination
- Monitor therapy effectiveness, identify optimization opportunities, and support quality improvement initiatives
Required Qualifications, Training, and Education
- Doctor of Pharmacy (PharmD) degree
- Licensed pharmacist in California
- Advanced practice pharmacist certification in California, meeting at least two specified criteria
- 3+ years of experience as a clinical pharmacist or completion of PGY2 residency
- Experience in clinical settings involving drug therapy management
