Job Summary

A company is looking for an Ambulatory Care Pharmacist who will enhance patient care through medication management and collaboration with healthcare teams.

Key Responsibilities
  • Provide comprehensive medication management, including reconciliation and dose optimization
  • Collaborate with healthcare providers to create individualized care plans and ensure medication coordination
  • Monitor therapy effectiveness and support quality improvement initiatives
Required Qualifications, Training, and Education
  • Hold a Doctor of Pharmacy (PharmD)
  • Must be a licensed pharmacist in California
  • Meet any two of the following: certification in a relevant field, completed a postgraduate residency, or provided clinical services for at least one year under a collaborative practice agreement
  • 3+ years of experience as a clinical pharmacist or completion of PGY2
  • Possess strong organizational and technical skills
