Job Summary
A company is looking for an Ambulatory Care Pharmacist who will enhance patient care through comprehensive medication management.
Key Responsibilities
- Provide comprehensive medication management including reconciliation and dose optimization
- Collaborate with healthcare teams to create individualized care plans and coordinate medication
- Monitor therapy effectiveness, identify optimization opportunities, and support quality improvement initiatives
Required Qualifications
- Doctor of Pharmacy (PharmD) degree
- Licensed pharmacist in California
- Certification in a relevant field of practice or completion of a postgraduate residency
- Minimum of 3 years' experience as a clinical pharmacist or completion of PGY2
- Experience providing clinical services under a collaborative practice agreement
