Job Summary

A company is looking for a CA Licensed Clinician: ASW, LCSW, APCC or LPCC.

Key Responsibilities

• Provides customized care to patients based on individual needs within assigned Care Pods
• Coordinates behavioral health care with primary care providers and monitors patient symptoms and treatment effectiveness
• Documents patient progress and treatment recommendations in electronic health records and participates in consultations and supervision sessions

Required Qualifications, Training, and Education

• Master's Degree in Social Work, Counseling, or a related field from an accredited program
• Licensed to practice in California as an associate clinician under supervision (ASW, APCC) or as an independent clinician (LCSW, LPCC)
• Minimum 1-5 years of experience as a licensed professional
