Job Summary
To provide comprehensive mental health support for individuals and families affected by neurodegenerative diseases, the full-time California Licensed Behavioral Health Manager will coordinate care, implement the Collaborative Mental Health Care program, and collaborate with medical providers and caregivers in a remote setting.
Key responsibilities
- Fulfill foundational responsibilities within the Collaborative Care Model, adhering to established guidelines
- Screen and assess patients for mental health disorders, facilitating engagement and follow-up care
- Deliver evidence-based behavioral interventions and monitor clinical symptoms, adapting care plans as necessary
Required qualifications
- Active independent clinical license (LCSW, LMFT, LPC, or equivalent)
- 0-2 years of clinical case or care management experience
- Strong organizational and interpersonal skills
- Passion for working with vulnerable populations and providing patient-centered care
- Background or clinical interest in neurodegenerative diseases preferred
