California Licensed Behavioral Health Manager

Job Summary

To provide comprehensive mental health support for individuals and families affected by neurodegenerative diseases, the full-time California Licensed Behavioral Health Manager will coordinate care, implement the Collaborative Mental Health Care program, and collaborate with medical providers and caregivers in a remote setting.

Key responsibilities

• Fulfill foundational responsibilities within the Collaborative Care Model, adhering to established guidelines
• Screen and assess patients for mental health disorders, facilitating engagement and follow-up care
• Deliver evidence-based behavioral interventions and monitor clinical symptoms, adapting care plans as necessary

Required qualifications

• Active independent clinical license (LCSW, LMFT, LPC, or equivalent)
• 0-2 years of clinical case or care management experience
• Strong organizational and interpersonal skills
• Passion for working with vulnerable populations and providing patient-centered care
• Background or clinical interest in neurodegenerative diseases preferred
