California Licensed Bilingual Case Manager

Job Summary

As a bilingual Lead Case Manager in the Los Angeles Metro area, the full-time hybrid position will manage comprehensive case management and mental health services for individuals with persistent mental illness and/or substance use, while working both in the field and remotely.

Key responsibilities

• Serves as the primary point of contact for members and their support teams, coordinating care and services
• Assesses member needs and develops comprehensive Care Management Plans with input from multidisciplinary teams
• Connects members to necessary social services and advocates on their behalf with healthcare professionals

Required qualifications

• Fluency in Spanish and a High School Diploma or GED; Bachelor's degree in a related field preferred
• Minimum of 2 years of experience working with Medi-Cal members in care management or mental health services
• Current California driver's license and acceptable driving record
• Familiarity with trauma-informed care and harm reduction practices
• Strong critical thinking and communication skills to engage effectively with diverse populations
