Job Summary
As a bilingual Lead Case Manager in a full-time hybrid role, the candidate will provide comprehensive case management, mental health, and substance use services to individuals with persistent mental illness and/or substance use, ensuring personalized care and support within an integrated healthcare setting.
Key responsibilities
- Serves as the primary contact for members and their support systems, coordinating care with a multidisciplinary team
- Assesses member needs and develops comprehensive Care Management Plans to address gaps in treatment and services
- Advocates for members, connects them to social services, and monitors treatment adherence while providing health promotion coaching
Required qualifications
- Fluency in Spanish and a High School Diploma or GED; a Bachelor's degree in a related field is preferred
- Minimum of 2 years of experience working with Medi-Cal members in care management or mental health services
- Current California driver's license and acceptable driving record
- Knowledge of community resources and familiarity with trauma-informed care and harm reduction practices
- Exceptional communication skills and the ability to engage diverse populations effectively
