California Licensed Bilingual RN Care Manager
Job Summary
Seeking a full-time California Licensed Bilingual RN Care Manager, who will manage a caseload of individuals with complex medical needs, support patient-driven care plans, and collaborate with a multidisciplinary team, all while working remotely during PST hours.
Key Responsibilities
- Support individuals in identifying health and wellness goals and incorporate them into Shared Care Plans
- Educate patients on medical and behavioral health conditions to enhance health literacy and provide medication reconciliation
- Collaborate with a multidisciplinary care team to address barriers to care and ensure all required assessments and screenings are performed
Required Qualifications
- Active Registered Nurse license issued by the state of California
- 2-3+ years of experience in care coordination or case management, preferably in a health plan or at-risk provider setting
- Bilingual in English and Spanish
- Strong technical skills and experience with CRM databases and virtual communication tools
- Demonstrated experience working with individuals facing complex chronic needs, homelessness, or Severe Mental Illness/Substance Use Disorder
