California Licensed Bilingual RN Case Manager

Job Summary

Seeking a California Licensed Bilingual RN Case Manager, the full-time remote position will manage health care coordination for members with complex and chronic care needs, focusing on assessing, developing, and implementing care plans to optimize member health.

Key responsibilities

• Ensures member access to appropriate health services based on their needs
• Implements and monitors care plans, facilitating necessary authorizations and referrals
• Collaborates with healthcare providers to develop and adjust care management treatment plans

Required qualifications

• Minimum of 5 years' clinical case management experience or equivalent education and experience
• Bachelor's degree in Nursing
• Current, unrestricted RN license in California
• Bilingual in Spanish and English
• Willingness to obtain RN licenses in NV, AZ, NC, TX (as applicable)
