Job Summary

A company is looking for a Remote Cardiometabolic RN (Full Time).

Key Responsibilities

• Deliver patient-centric health coaching integrating medical needs with patient preferences
• Triage patient-reported symptoms and side effects, escalating to providers when necessary
• Provide comprehensive care and education on diabetes, hypertension, hyperlipidemia, and weight management via chat, phone, and video

Required Qualifications

• Bachelor of Science in Nursing (BSN) required
• Active Registered Nurse license in good standing
• Certified Diabetes Care and Education Specialist (CDCES) or willingness to pursue certification
• 3+ years of direct patient care in an ambulatory or telehealth setting
• Proficiency with digital tools for care delivery
