California Licensed Care Coordinator

Job Summary

A company is looking for a Remote Care Coordinator (Bilingual Spanish, Medical Assistant, Must be in California).

Key Responsibilities

• Assist members with referrals, authorizations, and appointments through telephonic outreach
• Create cases and complete documentation in the Case Management module for discharges
• Collaborate with the RN Case Manager to manage care for members with complex needs

Required Qualifications, Training, and Education

• Minimum 1 year experience in Health Care, assisting members with authorizations and scheduling
• High School Diploma or GED; Bachelor's degree or four years additional experience in lieu of education
• Bilingual in English and Spanish
• Medical Assistant training or Medical Terminology training preferred
• Knowledge of Managed Care Plans and Medi-Cal preferred
