Job Summary

A company is looking for a Remote Care Coordinator LPN (Pacific Time Zone).

Key Responsibilities

Conduct monthly care coordination phone calls with patients and healthcare providers
Create and revise personalized care plans in collaboration with healthcare practices
Identify social determinants of health and coordinate resources for patient care

Required Qualifications

At least 1 year of experience as an LPN with active accreditation
Experience with multiple EMR/EHR systems and proficiency in Microsoft Office and GoogleSuite
Ability to maintain professionalism and compassion during patient interactions
Strong organizational skills and ability to work independently
Experience in call center, triage, or care coordination is a plus
